台灣縱貫越嶺步道第三段（北鎮段）的起點位於北橫公路處。由此起點下切溪谷，再向上沿著稜線穿過遙遠的山林區域。之後，往南通往司馬庫斯部落，接著沿著司馬庫斯古道下切到較深的溪谷後再往上通往鎮西堡。
如第二段（新北段）相同，此段步道屬於台灣中級山的困難地形等級，許多路段雜草叢生，連資深山友都會遇到困難。因此，建議山友必須具備基本定向能力。
Section 3 (or the Beizhen Section 北鎮段) of the Taiwan Peak-to-Peak Trail 台灣縱貫越嶺步道 begins at the Northern Cross-Island Highway 北橫公路. It immediately descends into the river valley to the south and then ascends again, following ridgelines through remote forested mountain areas. It then descends through the Atayal 泰雅族 indigenous village of Smangus 司馬庫斯 and into another deep valley, where it follows Smangus Historic Trail 司馬庫斯古道 and ascends again to its final destination of Zhenxibao 鎮西堡.
As with Section 2 (Xinbei Section 新北段), many of the trails in the section are overgrown and pass through the difficult terrain of Taiwan’s mid-elevation mountains 中級山. Basic orienteering skills are necessary throughout the section, and even experienced mountaineers will find progress difficult at times.
